Background Participants' understanding of clinical trials is important in informed consent. However, little is known about what information participants really want to know. Aims To demonstrate the existence of a discrepancy between participants' understanding and their desire to know. Methods The participants in clinical trials at Seoul National University Hospital were surveyed. The survey consisted of 11 statements based on the essential elements of informed consent. The participants gave two responses to each statement on a five-point Likert scale to rate (...) their subjective understanding and desire to know, respectively. Information discrepancy was defined as the difference between these two ratings: if understanding exceeded desire to know for a particular item, it was defined as ‘over-informed’; if desire to know exceeded understanding for a particular item, it was defined as ‘under-informed’. Results Participants reported good understanding of ‘voluntariness’, ‘duration’, ‘study involves research’ and poor understanding of ‘confidentiality’, ‘compensation’, ‘benefits’, ‘procedures’ and ‘risks or discomforts’. For ‘risks or discomforts’, ‘who to contact’, ‘voluntariness’, ‘duration’ and ‘procedures’, participants reported high desire to know compared with ‘confidentiality’, ‘purpose’, ‘study involves research’ and ‘benefits’. The elements ‘study involves research’, ‘voluntariness’, ‘duration’, ‘purpose’ and ‘who to contact’ were over-informed, while ‘compensation’, ‘risks or discomforts’, ‘procedures’, ‘confidentiality’ and ‘benefits’ were under-informed. Participants over 50 years of age, those without a college education and those whose participation was less voluntary were relatively less informed about the clinical trials. Conclusions An information discrepancy was observed between the participants' understanding and their desire to know. By putting more emphasis on under-informed elements, the quality of informed consent could be improved. (shrink)

BackgroundAlthough the Life-Sustaining Treatment (LST) Decision Act was enforced in 2018 in Korea, data on whether it is well established in actual clinical settings are limited. Hospital-acquired pneumonia (HAP) is a common nosocomial infection with high mortality. However, there are limited data on the end-of-life (EOL) decision of patients with HAP. Therefore, we aimed to examine clinical characteristics and outcomes according to the EOL decision for patients with HAP.MethodsThis multicenter study enrolled patients with HAP at 16 referral hospitals retrospectively from (...) January to December 2019. EOL decisions included do-not-resuscitate (DNR), withholding of LST, and withdrawal of LST. Descriptive and Kaplan–Meier curve analyses for survival were performed.ResultsOf 1,131 patients with HAP, 283 deceased patients with EOL decisions (105 cases of DNR, 108 cases of withholding of LST, and 70 cases of withdrawal of LST) were analyzed. The median age was 74 (IQR 63–81) years. The prevalence of solid malignant tumors was high (32.4% vs. 46.3% vs. 54.3%, P = 0.011), and the ICU admission rate was lower (42.9% vs. 35.2% vs. 24.3%, P = 0.042) in the withdrawal group. The prevalence of multidrug-resistant pathogens, impaired consciousness, and cough was significantly lower in the withdrawal group. Kaplan–Meier curve analysis revealed that 30-day and 60-day survival rates were higher in the withdrawal group than in the DNR and withholding groups (log-rank P = 0.021 and 0.018). The survival of the withdrawal group was markedly decreased after 40 days; thus, the withdrawal decision was made around this time. Among patients aged below 80 years, the rates of EOL decisions were not different (P = 0.430); however, mong patients aged over 80 years, the rate of withdrawal was significantly lower than that of DNR and withholding (P = 0.001).ConclusionsAfter the LST Decision Act was enforced in Korea, a DNR order was still common in EOL decisions. Baseline characteristics and outcomes were similar between the DNR and withholding groups; however, differences were observed in the withdrawal group. Withdrawal decisions seemed to be made at the late stage of dying. Therefore, advance care planning for patients with HAP is needed. (shrink)

It is widely thought that knowledge is factive – only truths can be known. However, this view has been recently challenged. One challenge appeals to approximate truths. Wesley Buckwalter and John Turri argue that false-but-approximately-true propositions can be known. They provide experimental findings to show that their view enjoys intuitive support. In addition, they argue that we should reject the factive account of knowledge to avoid widespread skepticism. A second challenge, advanced by Nenad Popovic, appeals to multidimensional geometry to build (...) a case where it seems intuitive that a person knows p even though p is false. In addition, Popovic argues that we should reject the factive account of knowledge because most of us would not become widespread skeptics if we discovered that ordinary objects in our world are actually four-dimensional. In this paper, we defend the factive account of knowledge against these arguments by challenging the intuitive appeal of the cases and arguing that there is no real threat of widespread skepticism for the factive account of knowledge. This paper examines Fichte's conception of the freedom of choice in Das System der Sittenlehre of 1798 as a solution to the dilemma posed by determinism and indeterminism. It show that Fichte does not simply affirm an indifferent power of voluntary choice, but demonstrates how such a power might co-exist with the measure of regularity and lawfulness we normally admit of human choices. Particular choices do not occur at random, but are based on general reasons. These reasons are in turn (...) based on the extent to which we have freely reflected on our original drive. Thus our choices can be comprehended and predicted from the point of reflection from which we choose. This does not lead to the denial of the freedom of choice, but only to rethinking it in terms of the freedom of reflection: Though we cannot choose other than what we choose from the point of reflection we occupy, we can choose other than what we choose by raising or lowering ourselves through reflection to a higher or lower point of reflection. In this work we investigate the Weihrauch degree of the problem Decreasing Sequence of finding an infinite descending sequence through a given ill-founded linear order, which is shared by the problem Bad Sequence of finding a bad sequence through a given non-well quasi-order. We show that $\mathsf {DS}$, despite being hard to solve, is rather weak in terms of uniform computational strength. To make the latter precise, we introduce the notion of the deterministic part of a Weihrauch degree. We then (...) generalize $\mathsf {DS}$ and $\mathsf {BS}$ by considering $\boldsymbol {\Gamma }$ -presented orders, where $\boldsymbol {\Gamma }$ is a Borel pointclass or $\boldsymbol {\Delta }^1_1$, $\boldsymbol {\Sigma }^1_1$, $\boldsymbol {\Pi }^1_1$. The best-known fact about the interaction of the Republican Roman poet Gaius Lucilius (c.180–103/102b.c.e.), the inventor of the genre of Roman verse satire, with the doctrine of Scepticism is probably a statement of Cicero: that Clitomachus the Academician dedicated a treatise to the poet (Cic.Luc. 102). Diogenes Laertius makes much of that writer's, Clitomachus’, industry (τὸ φιλόπονον, 4.67), with the comment: ‘to such lengths did his diligence (ἐπιμελείας) go that he composed more than four hundred treatises’. This phraseology surely reminds (...) those interested in Lucilius’ influence on later Latin poetry of Horace's disparaging comment,in hora saepe ducentos, | ut magnum, uersus dictabat(‘as a bravura display, he would often dictate two hundred verses in an hour’,Sat.1.4.9–10); moreover, Horace shortly afterwards calls his predecessorgarrulus atque piger scribendi ferre laborem(‘talkative and too lazy to bear the work of writing’, 1.4.12). Yet, a sceptical view of Horace's critique might have to think of Lucilius as hard-working, like his putative friend the Academic philosopher, Clitomachus. (shrink)

According to two-dimensional semantics, primary intension and secondary intension can play the role of reflecting the cognitive aspect of an expression like Fregean sense does. The aim of this paper is to argue that this role is likely a myth. To argue for this, I attempt to show that cognitive aspects of fictional names cannot be explained within the framework of two-dimensional semantics. To be more specific, I consider four ontological theories about fictional characters that two-dimensional semanticists might be tempted (...) to choose. And then, I argue that none of them can be combined with two-dimensional semantics in a satisfactory way. (shrink)

Does the relation of (actual) causation admit of degrees? Is it sensible to say, for example, that ‘as compared to his consuming the light beer, Clement’s consuming the moonshine was more a cause of his becoming drunk’? Suppose the answer is ‘yes’. Suppose also that country A unjustifiably ignites a lethal war with country B, and you intuit that, while most combatants of A are liable to lethal counterattack, most non-combatants of A aren’t similarly liable. Then, you might support your (...) intuition by reasoning as follows. ‘Perhaps most non-combatants of A causally contribute to A’s unjust, lethal war effort. However, unlike most combatants of A, their causal contributions are not of such a degree that makes them liable to lethal counterattack’. Such reasoning is rejected by Carolina Sartorio. This is due to the recent revealing of a certain puzzle, one which suggests to Sartorio that causation doesn’t come in degrees. Now, one motivation for Sartorio’s reaction to the aforementioned puzzle is her thought that we can, for the most part, ‘explain away’ the ‘illusion’ that causation comes in degrees. I’ll argue that Sartorio insufficiently supports her foregoing thought. Using Sartorio’s resources, we cannot (largely) ‘explain away’ the widespread appearance that causation comes in degrees. The book presents Fichte’s position on free will as a form of compatibilism that has not yet been explored in the literature. Due to early rationalist convictions, Fichte is as much concerned with reconciling freedom with a logical and a theological determinism as he is with a causal determinism. He sees in Kant’s novel concept of a pure practical reason a new form of rationalism, one consisting of a system of moral rather than natural necessitating grounds. At the same time, (...) he adopts a more radically libertarian stance on free will than Kant. Every member in a sequence of free actions is a “first and absolute member” and could be other than it is given the same antecedent natural events and natural laws. The interest of Fichte as a theorist of freedom lies in how he brings together freedom and predetermination in a way that challenges our assumption about their mutual exclusivity. The book provides an overview of Fichte’s philosophical system – the so-called “Doctrine of Science” – from 1793 to 1800 with the aim of contextualizing his theory of free agency and destiny. In doing so, it sheds light on how consideration of these issues in turn shapes the system. (shrink)

Many non-heteronormative Malaysian men find themselves on the receiving end of political, socio-cultural, and religious condemnations of their sexual identifyings and expressions. Their lived realities are often considered invalid, including from religious and theological perspectives. This article is a queer socio-theological project that examines the lived realities of six non-heteronormative Malaysian men who speak up and talk back on their sexualities and spiritual sensibilities. Using a Constructivist Grounded Theory Methodology, and aided by the theological musings of Marcella Althaus-Reid and a (...) document of the Federation of Asian Bishops' Conferences as analytical frameworks, this article analyzes and theologizes the complex processes of transcendent/desire, or negotiations of the “Profoundly More” and sexuality in the narratives of these men. This article suggests that transcendent/desire can be imaginatively assembled in three interrelated forms: as embodied connections with the transcendent; as a vocation; and as an imperative to flourish. In his article “Explanation Sketches” [3], Fred Newman argues that Scriven is mistaken in his interpretation of Hempel's notion of explanation sketch. According to Scriven, to complete an explanation is to fill in further initial conditions so that the explanandum of the sketch would be entailed by the new enlarged set of initial conditions. The point may be alternatively put by saying that according to Scriven to have a complete explanation is to have a covering-law which connects initial conditions with (...) the explanandum of the original sketch. Now Scriven's point against the deductive theory as applied to History is just that very often when we try to complete a given explanation sketch, we only succeed in unearthing a law which is either false or else trivial. Newman, however, contends that this is an excusable but, nevertheless, unsympathetic way of interpreting Hempel. He suggests an alternative account to the effect that to complete an explanation we need not necessarily produce a covering-law connecting initial conditions with the explanandum; all we need to do is to mention other initial conditions and certain laws which need not necessarily contain mention of the explanandum of the sketch. As he himself puts the point: “However, generously interpreted, when Hempel speaks of a sketch vaguely indicating laws we may take him as merely suggesting that some laws are vaguely indicated which would make up the explanans of an explanation of the event in question—not necessarily in the terms which describe the event in the sketch's explanandum.”. This paper is about the problem of explanation in anthropology. There are, broadly speaking, three theories of explanation, namely, the scientific theory, the historical theory, and finally what I have decided to call the phenomenological theory, after M. Natanson. The author argues that none of the three theories is adequate by itself to encompass the complex nature of anthropological science. The three theories correspond roughly to at least three different types of questions raised by anthropologists, and this being the case (...) the principle of methodological tolerance seems a natural and sensible principle to adopt. The paper also deals with the problem of reduction, i.e. the problem whether the three theories are different from and logically independent of one another. (shrink)

This article explores the Korean application of “mind-transmission” (K. chŏnsim, C. chuanxin) episodes to the intra-Sŏn (C. Chan) polemics. Korean Sŏn masters, unlike Chinese counterparts, sought for the religious meaning of the existence of multiple transmission episodes that circulated in East Asia from the Sŏn polemical perspective. In particular, Kagun and Paekp’a used the term “samch’ŏ chŏnsim” to promote their own visions of Sŏn within the situation in which different visions of Sŏn competed for dominance.

This paper will describe the rite for the exorcism of toch'aebi and examine its symbolic significance in the wider social reality of Cheju shamanism. Toch'aebi is a stranger deity who visits Cheju randomly and tries to get on good terms with the people. However, this deity afflicts people, particularly women, wearing down their vitality and causing a kind of “madness” (turida). The exorcism ritual of toch'aebi requires a sacrificial feast of roast pig and several days of dancing by the possessed. (...) For this reason, the exorcism ritual is called a dancing rite, or ch'unun kut. It is supposedly toch'aebi himself who has the desire to dance. Shaman's songs (seouje sori) are used to induce the toch'aebi, who is hiding in the patient's body, to dance and reveal his identity. The trance dance and singing are directed toward a dialogical construction of the demonic reality, which entails a romantic entanglement of toch'aebi and the possessed. By translating a history of personal suffering into the idiom of love, the shaman's songs develop a particular cultural logic of cure. Through an analysis of curing song texts, along with various modes of performance such as dance and comic play, this paper explores the transformative process in which a particular cultural logic of illness and cure are produced and enacted to reintegrate the patient into the social world. In practice, the analysis addresses the articulation of a personal case of madness that afflicted Sunho, a twenty-one year old female factory worker, whose social values and understanding are reflective of contemporary Korea. BackgroundCOVID-19 break out has created panic and fear in society. A strict kind of lockdown was imposed in Wuhan, Hubei province of China. During home confinement due to lockdown, people face multidimensional issues. The present study explored the psychological impacts of COVID-19 home confinement during the lockdown period and Wuhan’s residents’ attitude toward physical activity.MethodsA cross-sectional online survey was conducted to collect the primary data according to the study objectives. The population was Wuhan residents who were in home confinement. A (...) total of 2280 participant’s reply to the online questionnaire survey and collected data after quantifying the data, about 2200 ; about female, about male, others about, and about were not disclosed their gender participants responses were used for analysis. The collected data were analyzed through appropriate statistical techniques.ResultsAccording to results, H1 is supported with β = −40.793, t = 57.835, p = 0.000, which claimed a negative association between COVID-19 lockdown policy and behavior and attitude. Results for H2 reveals that the COVID-19 lockdown policy have negative influence on emotional control with β = −0.769, t = 46.766, p = 0.000 and it is supported. H3 documented a significant positive relationship between COVID-19 lockdown policy and lockdown period psychological impact, which means lockdown policy, is the main reason to increase the lockdown psychological impact. Further, COVID-19 lockdown policy have negative influence on physical activity and self-belief with β = −0.657, t = 32.766, p = 0.000 and β = −0.620, t = −6.766, p = 0.000 respectively. H6 stated that there is a positive impact of behavior and attitude toward physical activity. The results for H6, behavior, and attitude affecting the physical activity with β = 0.401, t = 10, p = 0.000, which is supported.ConclusionCOVID-19 home confinement created various psychological impacts, negatively affecting the emotional state due to depression and anxiety. Physical activity is the best strategy to manage human nature’s psychological issues, and people’s attitudes were positive toward physical activity during home confinement. However, the lockdown policy also affects physical activity participation negatively, and a sedentary lifestyle prevailed during home confinement. (shrink)

Christopher Nolan is the writer and director of Hollywood blockbusters like The Dark Knight and The Dark Knight Rises, and also of arthouse films like Memento and Inception. Underlying his staggering commercial success however, is a darker sensibility that questions the veracity of human knowledge, the allure of appearance over reality and the latent disorder in contemporary society. This appreciation of the sinister owes a huge debt to philosophy and especially modern thinkers like Friedrich Nietzsche, Sigmund Freud and Jacques Derrida. (...) Taking a thematic approach to Nolan's oeuvre, Robbie Goh examines how the director's postmodern inclinations manifest themselves in non-linearity, causal agnosticism, the threat of social anarchy and the frequent use of the mise en abyme, while running counter to these are narratives of heroism, moral responsibility and the dignity of human choice. For Goh, Nolan is a 'reluctant postmodernist'. His films reflect the cynicism of the modern world, but with their representation of heroic moral triumphs, they also resist it. (shrink)

How, and how well, do people switch between exploration and exploitation to search for and accumulate resources? We study the decision processes underlying such exploration/exploitation trade‐offs using a novel card selection task that captures the common situation of searching among multiple resources (e.g., jobs) that can be exploited without depleting. With experience, participants learn to switch appropriately between exploration and exploitation and approach optimal performance. We model participants' behavior on this task with random, threshold, and sampling strategies, and find that (...) a linear decreasing threshold rule best fits participants' results. Further evidence that participants use decreasing threshold‐based strategies comes from reaction time differences between exploration and exploitation; however, participants themselves report non‐decreasing thresholds. Decreasing threshold strategies that “front‐load” exploration and switch quickly to exploitation are particularly effective in resource accumulation tasks, in contrast to optimal stopping problems like the Secretary Problem requiring longer exploration. Auditory perception is traditionally conceived as the perception of sounds — a friend’s voice, a clap of thunder, a minor chord. However, daily life also seems to present us with experiences characterized by the absence of sound — a moment of silence, a gap between thunderclaps, the hush after a musical performance. In these cases, do we positively hear silence? Or do we just fail to hear, and merely judge or infer that it is silent? This longstanding question remains controversial (...) in both the philosophy and science of perception, with prominent theories holding that sounds are the only objects of auditory experience and thus that our encounter with silence is cognitive, not perceptual. However, this debate has largely remained theoretical, without a key empirical test. Here, we introduce a novel empirical approach, presenting experimental evidence that silence can be genuinely perceived (not just cognitively inferred). We ask whether silences can ‘substitute’ for sounds in event-based auditory illusions — empirical signatures of auditory event representation in which auditory events distort perceived duration. Seven experiments introduce three new ‘silence illusions’ — the one-silence-is-more illusion, silence-based warping, and the oddball-silence illusion — each adapted from a prominent perceptual illusion previously thought to arise only from sounds. Subjects were immersed in ambient noise interrupted by silences structurally identical to the sounds in the original illusions. In all cases, silences elicited temporal distortions perfectly analogous to the illusions produced by sounds. Our results suggest that silence is truly heard, not merely inferred, introducing a general approach for studying absence perception. (shrink)

In Schopenhauer’s thought, the will’s primacy over the intellect seems to suggest that the intellect plays no role in determining what we do. I provide an alternative picture of the intellect as actively deliberating and choosing in abstract cognition from what it passively receives from the will in natural cognition.

Lawrence Sklar in his book, Physics and Chance (1993), proposes a sophisticated account of reduction of thermodynamics (TD) by statistical mechanics (SM). I argue that Sklar's analysis of the alleged reduction of TD by SM is problematic in several respects. I consider a few counterexamples to show that none of what Sklar takes to be the central features of successful reduction in science (unification and identification) holds in the case of TD and SM. I suggest the broader conclusion that a (...) more useful way of understanding the relationship between TD and SM is as collaboration and competition among alternative methodologies rather than reduction of one theory to another. Goh, Michelle Christian discipleship is to live faithfully Jesus' commandment of love-of God and of our neighbour. The commandment to love especially those who are poor or in need was emphasised by Jesus in his actions and his teachings. Jesus' parable of the Good Samaritan has traditionally been an influential model of care and compassion. We are given an example of how we ought to extend ourselves to care for each other, especially those who are helpless, suffering or isolated.1 It (...) is increasingly recognised that elderly persons are amongst those who are in need in modern society. We study the computational content of various theorems with reverse mathematical strength around Arithmetical Transfinite Recursion (ATR_0) from the point of view of computability-theoretic reducibilities, in particular Weihrauch reducibility. Our main result states that it is equally hard to construct an embedding between two given well-orderings, as it is to construct a Turing jump hierarchy on a given well-ordering. This answers a question of Marcone. We obtain a similar result for Fraïssé's conjecture restricted to well-orderings.

This essay investigates what a nuanced and revisionary interpretation of Fichte’s Critical-idealist philosophy could reveal about its impact on the philosophic thought of Friedrich Schlegel. It argues that Schlegel sees the Wissenschaftslehre through the lens of the distinction Fichte famous draws between the “spirit” (Geist) and the “letter” (Buchstaben) of a philosophy. He considers the spirit of the Wissenschaftslehre to lie in its acute awareness of its own limitation as a work of art and its letter in the deductive or (...) demonstrative form it inevitably assumes upon being expounded. In the face of the discrepancy, Schlegel rejects the letter of the Wissenschaftslehre in favor of its spirit. (shrink)

No concept is more controversial in, and yet more central to, the Wissenschaftslehre than that of freedom. This chapter presents Fichte as an empirical indeterminist and a transcendental determinist. Underlying his better-known empirical account of freedom of voluntary choice, I argue, is a transcendental account of “freedom in itself.” The latter, given from the transcendental viewpoint, presents my world as a thoroughly determinate system of possible (inner and outer) experience, including the possible experience of what I can and will do (...) in it. From the perspective of transcendental genesis, freedom of voluntary choice is the result merely of my reflection on and analysis (rather than of my determination and synthesis) of the system. Additionally, I discuss other empirical expressions of freedom than free voluntary choice, including freedom of self-cultivation, freedom of thinking, external freedom and moral freedom. (shrink)
